LET GPS BE YOUR GUIDE TO SERVICES
FOR OLDER ADULTS

                   WHAT IS GPS?

GPS provides caregivers with needed answers on how to provide the best care for the older adults in their lives. GPS stands for Guidance, Planning and              Support.  Our Care Navigators are registered nurses who specialize in geriatric care. They provide personalized support, care planning and guidance to those who are caring for a loved one.

                    NEED TO FIND HELP?

GPS is especially helpful during times of transition, when a loved one:

  • is returning home after a hospital or rehabilitation stay
  • needs help planning for the future
  • has received a new diagnosis and/or medication regimen
  • has suffered a loss

 

GPS can assist caregivers who live locally or a long distance from their loved ones.

    

BASIC GPS SERVICE INCLUDES

For a reasonable fee, a Care Navigator makes a home visit which includes:

  • Assessment of physical and emotional health, support networks, and overall well-being
  • Review of medications and evaluation of regimen
  • Creation of a personal health record, medication list, and a Vial of Life (emergency info packet)
  • Evaluation of home safety
  • Assistance with advanced care planning
  • Support and resources for caregivers
  • Education on how to be a health care advocate
  • Coaching on how to communicate better with health care providers

 

The Care Navigator then provides the family with a written summary and plan regarding care options and available resources.

The Care Navigator continues contact with the family for up to one month regarding plan of care.

ADDITIONAL OPTIONS

For additional fees, GPS clients can also receive:

  • Medication management
  • Wellness visits by nurse
  • Nursing follow-up to activate plan of care
  • Social work support, including family, medication, caregiver support, senior living options
  • Nutritional counseling
  • Application assistance including entitlement/housing opportunities

 

In addition, older adults steered through GPS have direct access to the eleven other integrated services at SAGE Eldercare which can help improve their well-being and independence.
